Abstracts posters

Naast een aanbod van lezingen en workshops, wordt tijdens het congres ook de kans geboden actief te participeren tijdens de postersessie. Bij deze doen we een warme oproep voor uw bijdrage aan de hand van een abstract. Ook dit jaar zal een geldprijs worden uitgereikt voor de drie beste posters.

Presentaties van zowel lopend als afgerond onderzoek kunnen als poster ingediend worden. Abstracts worden ter goedkeuring voorgelegd aan het wetenschappelijk comité.

1)    Els Verheyen

Titel: Samen werken aan goede zorg – tips van personen met een eetstoornis en hun mantelzorgers

Probleemstelling: Personen met eetstoornissen en hun mantelzorgers signaleerden tijdens hun contacten met de zelfhulpgroep AN-BN tekorten in de hulpverlening bij eetstoornissen. Dit onderzoek wou in kaart brengen om welke tekorten het gaat, welke goede ervaringen mensen hebben en welke tips we hieruit kunnen distilleren voor de toekomst.

Methode: Gegevens werden verzameld via een online vragenlijst, een online focusgroep en twee real-life focusgroepen. De resultaten werden gecodeerd en verwerkt tot verschillende tips per doelgroep.

Resultaten: Deelnemers aan de online vragenlijst beoordelen het contact met hun laatste hulpverlener met een gemiddelde tevredenheid van 7/10 (N= 35). De spreiding is echter maximaal (tussen 1/10 en 10/10). Deze bevinding ondersteunt het belang van dit onderzoek naar de beleving van hulpverlening bij personen met een eetstoornis. Speciale aandacht voor de doelgroep met slechte ervaringen met hulpverleners is aan te raden.

Personen met een eetstoornis geven volgende tips aan lotgenoten: 1. Informeer jezelf, 2. Wees eerlijk, 3. Zoek zo snel mogelijk gepaste hulp, 4. Neem de regie voor je herstel zelf in handen, 5. Wees mild. Mantelzorgers geven lotgenoten (andere mantelzorgers) volgende tips: 1. Informeer jezelf, 2. Maak probleemsituaties bespreekbaar, 3. Vraag om feedback, 4. Stel je mondig op, 5. Behoud zelf je evenwicht. De tips die personen met een eetstoornis en familieleden meegeven aan hulpverleners hebben voornamelijk te maken met het opbouwen en in stand houden van de therapeutische relatie, de communicatie (verschillende aspecten), het belang van het erkennen van het perspectief van mantelzorgers, het gericht doorverwijzen en goed opvolgen en het erkennen van de ervaringskennis waarover men beschikt.

De resultaten van dit onderzoek werden gebundeld in de brochure “Samen werken aan goede zorg”.

Case-report van 19-jarig meisje met Down syndroom en anorexia nervosa

Probleemstelling:  Een 19-jarig meisje met Down syndroom en Hashimoto thyroïditis wordt verwezen naar een afdeling eetstoornissen binnen kinder- en jeugdpsychiatrie omwille van het beeld van anorexia nervosa. De volwassen psychiater vindt het moeilijk om ingang te krijgen omwille van de zwakke cognitieve mogelijkheden.
 Methode: aan de hand van de case-report worden verschillende aspecten belicht: de aanpak van een eetstoornis bij een volwassen vrouw, die echter door een mentale bemerking op niveau van een jong kind functioneert; hoe typisch anorectisch gedrag zich ontwikkelt bij iemand met een atypische persoonlijkheid voor het ontwikkelen van een eetstoornis; het samengaan van de somatische en psychiatrische zorg.
 Resultaat: De casus van deze jonge vrouw toont aan dat neurobiologische veranderingen tgv een sterk gewichtsverlies op korte tijd aanleiding geeft tot een typisch beeld van anorexia nervosa met angst om te eten, bewegingsdrang en preoccupatie met gewicht, ook bij iemand met beperkte mentale mogelijkheden. Psycho-educatie en een bekrachtigende directieve aanpak hielp deze patiënte om beter te worden. Een parallel traject kinderpsychiatrie – pediatrie was noodzakelijk gezien de gecombineerde problematiek van Down syndroom, Hashimoto thyroïditis en anorexia nervosa.

The impeding role of self-critical perfectionism on therapeutic alliance during treatment and eating disorder symptoms at follow-up in patients with an eating disorder

Abstract
Objective: This study examined the impeding role of self-critical perfectionism on therapeutic alliance during treatment (after 3 months of treatment) and eating disorder symptoms at follow-up (1 year later) in patients with an eating disorder. Although the adverse effect of self-critical perfectionism on therapeutic alliance in the treatment of depression is well established, this was never before investigated in the treatment of eating disorders.


Method: Participants were 53 females inpatients with a mean age of 21.1 years (SD = 5.5) treated for an eating disorder in a specialized inpatient treatment unit. Self-critical perfectionism was assessed at admission, therapeutic alliance was assessed during treatment, and eating disorder symptoms (drive for thinness, bulimic symptoms, and body dissatisfaction) were assessed at admission and follow-up. 
Results: Self-critical perfectionism was predictive of a less beneficial treatment alliance with the therapist. Although self-critical perfectionism was not directly predictive of changes in eating disorder symptoms, it was indirectly related to less reductions in body dissatisfaction through the therapeutic alliance.


Discussion: These results point to the importance of self-critical perfectionism in patients with an eating disorder on the therapeutic alliance and in turn on changes in eating disorder symptoms. Treatment implications are discussed.

Obesitas en eetgedrag: Resultaten uit de praktijk

Probleemstelling

Het verkrijgen van inzicht in het effect van het behandelprogramma van CO-EUR op het gebied van lichaamsmetingen, eetgedrag en kwaliteit van leven in de obese (BMI > 30 kg/m2) klantenpopulatie.

CO-EUR (Centrum voor obesitas-eetstoornissen Europa) biedt een multi-disciplinaire behandeling voor mensen met obesitas in combinatie met een eetstoornis.

Het oorspronkelijke behandeltraject duurde 18 maanden, met elke 3 maanden een (medische) controle. De gegevens op deze poster horen nog bij de 18 maanden behandeling. Inmiddels biedt CO-EUR een drietal behandeltrajecten die met modules een behandeling op maat bieden voor elke cliënt.

Methode

Mensen met een BMI > 30 kg/m2 en ouder dan 18 jaar zijn geïncludeerd in deze studie. De resultaten van de 18 maanden behandeling van 426 klanten zijn geanalyseerd aan de hand van een intention-to-treat analyse. Elke 3 maanden zijn lichaamsmetingen gemeten, elke 6 maanden worden vragenlijsten afgenomen.

Resultaten

Na 18 maanden behandeling realiseerden de klanten gemiddeld 10,2 ± 24,1% (p < 0.001) gewichtsverlies, de BMI daalde gemiddeld 3,9 ± 3,9 kg/m2 (P < 0.001) en de buikomvang nam met gemiddeld 13 ± 13 cm (p < 0.001) af. Alle scores op de Three Factor Eating Questionnaire (TFEQ) verbeterden significant (p < 0.001) al na 6 maanden behandeling en dit resultaat werd gecontinueerd tot op 18 maanden behandeling. De algemene gezondheid score uit de SF-36 vragenlijst verbeterde significant met 4,8 schaalpunten (p < 0.001) na 18 maanden behandeling.

Title: The role of insecure attachment and maladaptive emotion regulation in adolescent girls with restrictive Anorexia Nervosa

Introduction. Previous research has found an association between insecure attachment and eating disorder symptoms. However, little research has been conducted on clinical groups of adolescents and little is known about how insecure attachment might be linked to eating disorder symptoms. Based on the interpersonal vulnerability model  and the emotion regulation model of attachment, maladaptive emotion regulation might be expected to be an important mediating factor.  Therefore, the present study aims to investigate whether insecure attachment towards the primary attachment figure (mother) is associated with eating disorder symptoms in a clinical group of adolescents, and whether these associations are mediated by maladaptive emotion regulation strategies.

Method. Data was collected through two specialised adolescent eating disorder units in Belgium and is still running today. The current sample consists of 42 adolescent girls (M age=14.39), with restrictive Anorexia Nervosa. Patients filled in valid and standardised questionnaires concerning attachment patterns towards mother, emotion regulation strategies and eating disorder symptoms. Results. Mediation analyses were performed using bootstrapping procedure. Attachment anxiety had a significant direct effect on eating disorder symptoms, whereas attachment avoidance did not. Moreover, the direct effect of attachment anxiety disappeared when taking account for maladaptive emotion regulation, thereby indicating a mediation effect. However, no mediation effect was found for attachment avoidance. These preliminary results are partially in line with the suggested mediation models of the interpersonal vulnerability model  and the emotion regulation model of attachment. 

Discussion. Further research should replicate and elaborate on the findings of the current study and if the suggested mediation models can be replicated,  treatment of eating disorders should focus on the attachment style of adolescents, as well as on their maladaptive emotion regulation strategies.

Ouders, zorgverleners en medepatiënten als motiverende coaches: Autonomie-ondersteuning bij patiënten met een eetstoornis

De Zelf-Determinatie Theorie (ZDT) stelt dat de ondersteuning en bevrediging van de drie psychologische basisbehoeftes aan autonomie (d.w.z.,  handelen in overeenstemming met de ware ‘zelf’), competentie (d.w.z., zich als succesvol ervaren), en relationele verbondenheid (d.w.z. een warme band ervaren) de motor vormt voor onze psychologische ontwikkeling en optimale motivatie. In deze longitudinale studie bij een groep patiënten met een eetstoornis (N = 84) wordt de invloed van autonomie-ondersteuning vanuit zowel ouders, zorgverleners als medepatiënten op de behoeftesatisfactie en motivatie van patiënten onderzocht. Een autonomie-ondersteunende aanpak houdt het bieden van keuzemogelijkheden in, het geven van een zinvolle uitleg en het werken vanuit het perspectief van de patiënt. De relevante variabelen zijn op drie momenten gemeten, namelijk in het begin van de behandeling, na 14 dagen en aan het einde. Resultaten toonden aan dat autonomie-ondersteuning vanuit deze drie cruciale sociale figuren bijdroeg aan ervaren behoeftesatisfactie. Behoeftesatisfactie relateerde aan meer autonome motivatie. Bij anorexiapatiënten zagen we tevens dat deze meer autonome motivatie gepaard ging met een toename in BMI tijdens therapie. Samenvattend kunnen we stellen dat motivatie een dynamisch construct is wat positief beïnvloed kan worden door een autonomie-ondersteunende aanpak.

The role of specific environmental factors and self-esteem in the association between temperament and eating problems.

Introduction. The traits Sensitivity to Punishment (SP) and  Sensitivity to Reward (SR) are often linked to eating disorders (EDs) (e.g. Harrison, O’Brien, Lopez & Treasure, 2010). However, an important gap in current literature is the abstinence of studies examining possible moderating and mediating factors in this association. For example, one possibility is that when the environment is perceived as very demanding, this affects a patient more compared with growing up in a warm family climate. Secondly, previous research has shown that temperament might be associated with self-esteem (Heimpel, Elliot, & Wood, 2006). In turn, self-esteem is known to play a crucial role in EDs (e.g. de Santana, Ribeiro, Giral, & Raich, 2012) and, as such, it might play a mediating role in the association between temperament and eating behaviour.

Method. An explorative study was performed in a clinical sample of 162 patients (93.8% female) between the ages of 15 and 60 years (M=24.14) with an ED-diagnose. Through hierarchic linear regression we examined the moderating role of parental criticism and high expectations (MPS-F; Frost, Marten, Lahart, & Rosenblate, 1990) and the mediating role of self-esteem (RSES; Rosenberg, 1965) in the association of SP and SR (SPSRQ; Torrubia, Avila, Molto, & Caseras, 2001; BIS/BAS Scales; Carver & White, 1994) with external eating, emotional eating and dieting (DEBQ; van Strien, Frijters, Bergers, & Defares, 1986), with concerns over eating, body shape and weight (Con) (EDE-Q; Fairburn & Beglin, 1994) and with drive for thinness (DT) (EDI-II; Garner, 1991).

Results. Results revealed that higher parental criticism and expectations combined with higher SP and SR was related to more external eating. High parental criticism and expectations also interacted positively with SR in predicting emotional eating. Self-esteem mediated the relation between SP and both Con and DT, with a negative relation between SP and self-esteem as well as between self-esteem and both Con and DT. These results may enhance our insight into the way temperament is linked with eating behaviour and stress the need for further research in this area.

A continuum of neuropsychological  functioning across the diagnostic spectrum of eating disorders?

 Objective: Anorexia nervosa (AN) as well as bulimia nervosa (BN) has been associated with weaknesses in central coherence (CC) and set shifting (SS) abilities. These cognitive deviations, compared to controls, may point to prefrontal cortical disturbances. However, it remains unclear whether or not similar top-down control mechanisms are present in AN and BN, when compared to healthy participants. Consequently, the main aim of the current study was to investigate cognitive similarities and differences across eating disorder diagnostic categories on those features.

Methods: 19 female patients with BN completed five neuropsychological tests (Block Design, Object Assembly, an adapted task-switching paradigm, Wisconsin Card Sorting Test and Trail Making Test). Scores were compared with earlier reported data on AN restrictive type (N=29), AN bingeing-purging type (N=19) and healthy control participants (N=26).

Results: On CC measures, no differences could be established between AN-BP and BN patients, while AN-R patients performed significantly worse than AN-BP patients and HC. On SS, no group differences were observed.

Discussion: The performance of AN-BP patients seems to resemble BN patients’ performance more than AN-R patients’ performance on CC tasks. The last groups performs worse. These findings are suggestive for a continuum in neuropsychological functioning across the eating disorder spectrum.

Erythema ab igne in an adolescent Anorexic female

INTRODUCTION
Erythema ab igne (EAI), first described by the German dermatologist Abraham Buschke as Hitze melanosis ( melanosis induced by heat), is a reticulated, localized, net or spider-like erythema with hyperpigmentation of the skin. It is due to repeated and prolonged exposure to a heat source. Multiple heat sources have been reported to cause erythema ab igne: heat blankets, heating pads, hot water bottles, laptop computers and different other objects. The clinical presentation is with an asymptomatic reticular, pigmented dermatosis with sharp margins, rarely presenting as bullous lesions and without squamation. Immediate removal of the heating source is curative. Complete remission is seen in early-stage lesions, while in chronic exposure changes may become permanent and necessitate cosmetic surgery. Following longstanding exposure, precancerous lesions and malignancies have been described.
CASE REPORT
A 16.2- year old female was admitted with a weight loss of 36.6% in a 6 months period. She was complaining of an extreme feeling of general cold and fatigue, was constipated and reported postprandial fullness with epigastric discomfort. She admitted to have anorexia nervosa of the restrictive type. Her personal history included hospitalisation for mandibular reconstruction 1 year before. Her physical examination was significant for a weight of 52 kg, height of 176.5 cm , BMI of 16.7 kg/m² (BMI before the eating disorder was 25.7 kg/m²), an oral temperature of 34.5°C, a blood pressure of 87/56 mm Hg, a pulse rate of 41 bpm and an oxygen saturation of 100% in room air. Her heart sounds were regular and bilateral lung auscultation was clear. The extremities demonstrated acrocyanosis without pitting edema, but with distal livedo reticularis of the legs. Around the umbilicus we observed a reticulate erythematous spider-like pattern without squamation. A similar less pronounced net-like pattern was observed on both thighs (right more than left). She admitted to quasi persistent use of heat pads, to reduce a general feeling of cold and to improve her gastrointestinal complaints. The diagnosis of erythema ab igne was retained. Following advise to stop every heat exposure to the skin, 2 weeks later the erythematous lesions became hyperpigmented.
RESULTS
Clinical records of this patient were compared with a pubmed search about erythema ab igne in patients with an eating disorder.
CONCLUSION
Erythema ab igne is described in patients with an eating disorder. Facing an increased pain threshold, prolonged heat exposure to treat a general feeling of cold, can induce this dermatosis.

Is there a link between ferritin and the cardiac function in anorexic adolescents?

INTRODUCTION
Very little is carried out on iron and hematological status in AN patients. Normal serum iron and elevated serum ferritin concentration were the most revelant results seen in few studies based on small cohorts of AN patients. In cardiac literature an overload of iron stores raises the risk of ischemic heart disease. Because concentrations  of serum ferritin are directly proportional to intracellular ferritin concentrations, it is considered to be the best clinical measure of body iron stores and we used this parameter in our study . Iron overload has been found to increase vascular oxidative stress and accelerate arterial thrombosis by  promotion of oxidized low-density lipoprotein and direct endothelial injury.
AIM
 The aim of this study was to compare in the acute state of undernutrition, biochemical, endocrine and cardiac parameters between female anorexic adolescents with a high ferritin and a normal ferritin.
METHOD
 The study has a prospective design as all patients with anorexia nervosa (AN) admitted to our Eating Disorder Unit from 2002 till 2012 were scheduled to undergo a full clinical examination, biochemical and endocrine bilan together with a cardiac examination (Echocardiographic evaluation + 12 lead ECG). In total 311 adolescent girls ( 9.8-17.87 years) were referred during the current study and all fulfilled the DMS-IV (Diagnostic and Statistical Manual of Mental disorders, 4th edition DSM-IV ) criteria ascertained by a child psychiatrist and a paediatric nutritionist. None of the patients had a family history of cardiac diseases or of any systemic disease involving the cardiovascular system. Laboratory and clinical parameters: Blood samples were drawn for: BUN, creatinine, electrolytes, cholesterol, thyroid hormones, insulin growth factor-1, iron, ferritin, zinc, hemoglobin, hematocrite, leukocytes,fibrinogen and liver tests.                                                    Body mass index (kg/m²) was calculated and curves and centiles were used as reported by Rolland-Cachera et al.(1991).
RESULTS
 In total we identified a high ferritin level > 137 ng/ml in 82 of the 311 in the anorexic girls (26,4%). Risk factors for developing a high ferritin are: BMI: 14.41 ± 1.59 kg/m²; HR: 56.01 ± 16.36 bpm; LVM/height 2.7: 23.99 ± 5.25 g/m 2.7; cholesterol: 192.65 ± 50.70 mg/dl; AST: 31.58 ± 12.11 U/L; ALT: 43.93 ± 47.67 U/L, IGF-1: 113.16 ± 74.97 ng/ml and  FT3: 3.08 ± 1.80 pmol/L. The euthyroid sick syndrome was found in 33% of the patients with high ferritin versus 18.3% in the normal ferritin group.
CONCLUSION
 A higher ferritin level is correlated with a higher cholesterol level and lower FT3 level. Both parameters are cardiovascular risk factors for atherosclerosis. Further research is needed.

Predictors of Reduced Corrected Left Ventricular Mass in Female Anorexic Adolescents.

Background and aim: Decreased left ventricular mass in anorexia nervosa (AN) is amply reported. The aim of this study was to identify predictors and risk factors of reduced LVM/ height 2.7 (c LVM)  in a cohort of adolescent anorexic females.

Methods: Retrospective study of all anorexic girls referred to our tertiary eating disorder unit        (September 2002-December 2012), for somatic assessment of weight loss. All fulfilled DMS-IV criteria, without a family history of cardiac disease or any disease involving the cardiovascular system.

Results: 267 AN girls (age: 14. 63 ± 1.65 y) ( BMI: 15.72 ± 1.81 kg/m²) were included. Ferritin, weight loss, IGF-1, and total cholesterol were independent statistically significant predictors of cLVM  (p < 0.001).                                                                                                                                                                                                                                                                         
Conclusion: Decreased cLVM  is common in anorexic girls, especially with severe weight loss. Four factors predicted decreased cLVM  in our anorexic population: ferritin, weight loss, IGF-1 and total cholesterol level.

Tijdens de middagpauze is er gelegenheid tot het bezichtigen van de posters en tot het stellen van vragen. Posters worden beoordeeld door het wetenschappelijk comité. De uitreiking van de posterprijzen gebeurt om 12u.30.
